We followed a Writer Agent recommendation that later turned out to be based on outdated assumptions. When the outcome was poor, no one wanted to take responsibility because it was “what the agent advised.” How should accountability work here?

Accountability must always stay with humans. Agents execute logic, but people approve and act. If responsibility is shifted to the system, organizations lose the ability to learn from mistakes.

This is why governance frameworks should clearly define decision ownership. High-impact decisions should require explicit human sign-off, regardless of agent confidence.
